The TCA drug test detects tricyclic antidepressants (TCAs), a class of medications used mainly for depression and some types of pain. It is ordered to check whether the amount of TCA in the blood is within a safe range, especially when side effects or overdose are suspected.

What it measures

  • Total tricyclic antidepressants — A combined measurement of drugs such as amitriptyline, nortriptyline, imipramine, and desipramine.
  • Amitriptyline — A common TCA often prescribed for depression or nerve pain.
  • Nortriptyline — The active form of amitriptyline, also used as its own medication.
  • Imipramine — An older TCA that may still be prescribed.
  • Desipramine — A TCA related to imipramine.

Who it is for

A doctor or psychiatrist may order this test when a person takes a TCA and has concerning symptoms, or when an overdose is suspected. It may also be used to check whether a prescribed dose is producing a safe blood level.

Preparation

No special preparation is usually needed. Tell your healthcare provider about all medications and supplements you take, because some can affect the result.

Results and what they mean

Results are reported as a number for each drug or as a total level, along with the laboratory's reference range. Your clinician interprets the level in the context of your dose, symptoms, and other factors. This is general information only and not medical advice; always discuss your results with a qualified healthcare professional.

Frequently asked questions

Is this test the same as a urine drug screen?

No. This test uses a blood sample and measures the actual amount of tricyclic antidepressants in your blood, not just whether they are present.

How long does it take to get results?

Results are typically available within 1 to 3 business days.

Can I eat or drink before the test?

Yes, fasting is not required unless your healthcare provider gives you other instructions.

What if my level is outside the reference range?

A level outside the range does not automatically mean there is a problem. Your clinician will consider your dose, symptoms, and overall health to decide what it means for you.
